Your First Visit, Step by Step
Get Ready for Your First Visit With Sea Breeze Psychiatry
Sea Breeze Psychiatry is accepting new patients, and getting started takes three steps. You’ll schedule your visit, sign up for the patient portal, and download your forms. This page covers those steps, what to bring, and what to expect from in-person and telehealth visits.

Three Steps to Get Started
How to Become a New Patient
To become a patient, you’ll schedule a visit and finish your paperwork online.
- Step 1, schedule:Request an appointment online for an office visit in Miramar Beach or a telehealth visit.
- Step 2, sign up for the patient portal: Create your patient portal account, where you’ll find your forms.
- Step 3, download your forms: Download your forms through the portal and fill them out before your visit.
What to Bring With You
What to Bring and When to Arrive
For in-person visits, please arrive 15 minutes early so you have time to finish registration before you meet your provider.
Please bring these items with you:
- Insurance card: Your current insurance card, so our team can update your information.
- Photo identification: A valid photo identification card.
- Medication list: A list of your current medications.
- Copay: Your office copay, the set amount your plan asks you to pay at each visit.
Forms Ready Before You Arrive
Patient Forms and the 24-Hour Rule
Please finish your forms at least 24 hours before your visit so your appointment can go ahead.
The Authorization and Consent for Treatment form is required for all patients. It covers your consent for treatment and your agreement to financial responsibility, along with how we may contact you by phone, email, or text.
For a telehealth visit, you’ll also complete the Informed Consent for Telehealth Services form. It explains how your information is recorded, kept, and sent during virtual visits.
Other forms let you allow us to share your medical information with a person or facility you name. You can find the full set, including several forms in Spanish, on our Patient Forms page.
Care From Wherever You Are in Florida
What to Expect From a Telehealth Visit
Telehealth visits are open to patients who live in Florida. You don’t need an in-person visit to get started.
Dr. Larry De Lay sees patients by telehealth as well as in person. Sarah Miller, a psychiatric-mental health nurse practitioner (PMHNP), sees patients by telehealth only.
Our appointments run on Central Time (CT), which is one hour behind Eastern Time. If you live in an Eastern Time part of Florida, keep that difference in mind when you schedule.
A Little Flexibility for Late Arrivals
If You’re Running Late
Our office tries to stay on schedule. If you arrive late, we’ll see you the same day when we can. You may have a wait, or we may need to reschedule your visit.
